For the proper treatment of your animals and the corresponding billing, it is necessary to store certain
personal data. We therefore request your consent.
As a pet owner, I agree that the veterinary practice may store and use the following data:

• My address
• My contact details (e.g. telephone number, email address)
• Data related to the treatment of my animals
• Records of treatments and medications administered

I agree that my data may be shared, where necessary, with:

• External laboratories
• Billing service providers
• Pet registries (e.g. TASSO)
• Other veterinary practices (referrals)
• External communication service providers

Further disclosure only occurs within legal requirements and with renewed consent if necessary.
I may request information about stored data at any time. This consent is voluntary and may be revoked at
any time. Data will then be deleted.

Owner Declaration and Agreement

I confirm that I am the owner of the animal and authorized to approve treatment. I am willing and able to
bear the costs incurred.
If I am not the owner, I confirm I act with authorization. Otherwise, I accept financial responsibility.
I authorize third-party services (e.g. laboratory diagnostics) if required.
I confirm that my information is accurate and authorize examination/treatment. Payment is due
immediately after treatment unless otherwise agreed.
