The storage of certain personal data is required to provide proper medical care for your animals and to process billing. We kindly ask for your consent below.

As the pet owner, I agree that Tierärzte mit Herz GmbH (a company of Schoenert GmbH) may store and use the following data for the purpose of providing the best possible care:
- my home address,
- my contact information (e.g., phone number, email address),
- data related to the treatment of my animals, and
- curative treatments performed and medications administered.

I agree that my data may be shared, where necessary, with:
- external laboratories,
- other veterinary practices in the case of a referral,
- pet registries (e.g., TASSO),
- Schoenert GmbH (parent company) for the management of the digital patient record via tierarzt.ai and for administrative purposes,
- payment service providers for processing cashless payments: Klarna Bank AB (Sweden), Stripe Inc. (USA), GoCardless Ltd. (UK),
- communications service providers: Twilio Inc. (USA), Meta Platforms Ireland Ltd. / WhatsApp (USA) – only at the initiative of the pet owner,
- Hetzner Online GmbH (Germany) as the hosting provider for the practice website and infrastructure,
- Google LLC (USA) as part of the server infrastructure used,
- Supabase Inc. – database services and server-side functions; server location: EU (Frankfurt).

Notice regarding data transfers to third countries: Some of the service providers listed above are located outside the EU/EEA (in particular the USA and UK). Data transfers are carried out on the basis of EU Standard Contractual Clauses pursuant to Art. 46(2)(c) GDPR. Data Processing Agreements pursuant to Art. 28 GDPR are in place with all service providers.

Beyond the above, data will only be shared with third parties within the limits of applicable law, i.e., only with my renewed consent where required.

I agree,
- to be informed by the practice about upcoming preventive care reminders and vaccination appointments for my animals,
- to be contacted with current practice news and information by phone, email, and/or WhatsApp,
- that this consent applies equally to Schoenert GmbH as the parent company, insofar as contact is made in connection with care provided by the practice or related services.

I may request information about the scope of stored data at any time.
I have been informed that this declaration is voluntary and may be revoked at any time. Revocation does not affect the lawfulness of processing carried out prior to the revocation. In the event of revocation, data already collected will be deleted, unless statutory retention obligations apply.

I confirm that I am the owner of the animal and therefore authorized to enter into a contract for the performance of necessary treatments and surgical procedures. I further confirm that I am willing and able to cover the costs incurred. I hereby declare that I am not currently subject to any judicial debt proceedings. If I am not the owner of the animal, I confirm that I am acting on the explicit instruction of the owner. If no such authorization exists or if the owner disputes it, I confirm that I will be personally responsible for all costs arising from the treatment. To the extent necessary for diagnosis, I authorize you to obtain third-party services (such as laboratory tests) in my name and at my expense.

I confirm the accuracy of the information I have provided and hereby commission you to examine and/or treat my animal, confirming that I am authorized to do so.

Payment: I acknowledge that all services are to be paid cashlessly and immediately upon invoicing. Accepted payment methods are: debit card (EC-Karte / Girocard), credit card (Visa / Mastercard), instant bank transfer via Klarna Pay Now, and direct debit via GoCardless. Installment payment via Klarna is only available if requested and approved by Klarna following a credit check before the start of treatment. In the event of default, a formal court payment order will be filed immediately after notice; all resulting court and procedural costs will be borne by the client. The practice does not work with debt collection agencies. Subsequent treatments are also subject to this agreement.

Appointments: I acknowledge that scheduled appointments are binding. Cancellations must be made at least 48 hours in advance (business days, Monday through Friday). In the event of a late cancellation or no-show, the practice reserves the right to charge a cancellation fee of 30% of the estimated treatment costs. For regular appointments, the minimum cancellation fee is $30.00 and the maximum is $100.00, depending on the type and scope of the appointment. For surgical procedures (OPs), the minimum cancellation fee is $100.00.
