Consent to the transmission of information by e-mail, SMS and postal mail and to the processing of personal data for an unlimited period of time

I agree that the veterinarian may process my email address and other data provided in the registration form for an unlimited period of time for the purpose of providing information about veterinary services ( maximum once a month ) and/or reminders about appointments, such as vaccinations or deworming. I can revoke my consent at any time with effect for the future by notifying the veterinarian by e-mail or letter free of charge. The legality of the processing carried out on the basis of the consent until the revocation is not affected by the revocation.


Information on data processing according to the General data Protection Regulation ( DSGVO - Datengrundschutzverordnung )

1. Processing of personal data

Personal data will only be processed by us if it is permitted by law or if you consent to the data processing of your personal data. We process your personal data in particular within the framework of a legal permission on the legal basis of Article 6 (1) (b) DSGVO , in order to fulfill a treatment contract with you , or to carry out necessary pre-contractual measures, which follow your request . If you have consented to the sending of information by e-mail, the data processing will be carried out on the legal basis of Article 6(1)(a) DSGVO.

Accordingly, the purpose of processing your personal data is to process your requests for treatment, to maintain the necessary medical records in the event of treatment, for documentation required by law and for billing our services. For these purposes, these data may also be passed on to third parties on the legal basis of Article 6(1)(b), which we absolutely need to fulfill the treatment contract ( e.g. referral practices/ clinics, laboratories, etc.).

If you have consented to receive information by e-mail, the purpose of the data processing is to send you information about veterinary services and/or reminders about appointments, such as vaccinations or dewormings.

Your personal data will only be passed on to other third parties if we are legally obliged to do so, in order to assert legal claims, to investigate criminal offences that may have been committed or if you expressly consent to your personal data being passed on.


2. Right of withdrawal of consent

You may revoke any consent expressly given to us at any time with effect for the future. The legality of the processing carried out on the basis of the consent until the revocation is not affected by the revocation.


3. Storage and deletion periods for personal data

If the purpose of processing your personal data no longer applies (complete processing of the treatment contract), your personal data processed by us will be deleted or initially blocked in accordance with the following regulations.

If individual data must be retained for verification purposes and/or due to legal retention periods, the deletion shall be replaced by the blocking of the data. The data to be retained may then be processed exclusively for the aforementioned purposes.

Retention requirements under tax law stipulate a retention period of 10 years for invoice data and 6 years for other documents that are important for taxation. The retention periods start to run at the end of the calendar year in which the invoice was created or the other data was received.

Provided that you have consented to the processing of your personal data for an unlimited period of time, we process your personal data for the purposes specified in the consent for an unlimited period of time. If you cancel your consent, your personal data will be deleted or blocked in accordance with the above-mentioned regulations.The legitimacy of the processing based on the consent until the revocation is not affected by the revocation

6. Automated decision making including profiling

Automatic decision making including profiling pursuant to Article 22(1) and (4) does not take place.

I confirm that I am the owner of the animal and therefore authorized to conclude a contract for the required treatments and operations. I further confirm that I am willing and able to bear the costs incurred. In this regard, I declare that I am not in any legal debt proceedings at the time of this declaration. If I am not the owner of the animal, I declare that I am acting on behalf of the owner of the animal. In the absence of an authorization or if the animal owner denies an authorization, I hereby confirm that I will be responsible for the costs of the treatment. If necessary for the diagnosis, I authorize you to use the services of third parties (laboratory tests, etc.) in my name and at my expenses.

I hereby confirm that the information I have provided is correct and authorize you to examine or treat my animal and assure you that I am authorized to do so. I assure that I am authorized to do so. I confirm that I am aware that I will be required to pay the treatment costs incurred in cash, by EC card or by credit card after my pet has been examined. Subsequent treatments are also subject to this contract.

If an appointment is repeatedly not canceled at least 24 hours in advance, we may charge a cancellation fee of 30.00 EUR. The time slot is only blocked for your pet and we do not double book appointments, which saves you waiting time. However, we cannot compensate for any cancellations. We ask for your understanding and cooperation.
