If you would like to book an appointment for more than one animal, please first make the booking for just one animal. Once you have completed your appointment booking, you will then have the opportunity to book appointments for additional animals.

For the proper treatment of your animals and for billing purposes, it is necessary to store certain personal data, for which we kindly ask for your consent below.

As a pet owner, I agree that the veterinary practice may store and use the following data for the best possible care:
• my address,
• my contact information (e.g., phone number, email address),
• data related to the treatment of my animals, as well as
• curative treatments performed and medications administered.

I agree that my data may be shared, as necessary, with:
• external laboratories,
• external service providers necessary for billing (e.g., billing agencies),
• pet registries (e.g., TASSO),
• other veterinary practices in case of a referral, and
• external service providers for communication with the practice.
Further sharing of data with third parties will only occur within the legal framework, i.e., possibly only with my renewed consent.

I agree to be:
• informed by the practice about upcoming vaccinations for my animals, and
• kept up to date with current information from the practice.

I can request information about the extent of the stored data at any time.
I have been informed that this declaration is voluntary and can be revoked by me at any time. In this case, any data already collected will be deleted.

I confirm that I am the owner of the animal and therefore authorized to enter into a contract for the performance of necessary treatments and surgeries. I further confirm that I am willing and able to bear the costs incurred. I declare that I am not currently involved in any court-ordered debt proceedings. If I am not the owner of the animal, I assure that I am acting on the explicit instructions of the animal’s owner. Should I lack authorization or if the owner disputes the authorization, I hereby confirm that I will cover the costs arising from the treatment. Insofar as it is necessary for diagnostic purposes, I authorize you to use third-party services (such as laboratory tests) in my name and at my expense.

I hereby confirm the accuracy of my statements and authorize you to examine and/or treat my animal, assuring you that I am authorized to do so. I acknowledge that I must pay the incurred treatment costs in cash, by debit card, or by credit card after the examination of my animal. Follow-up treatments are also subject to this contract.

By signing, I agree to the treatment contract and the outlined data protection consent declaration within the scope of its intended purpose.
