Data Protection Notice

Vierbeinerphysio – Dr. Laura Klingseisen-Bruun processes the personal data required to perform the treatment contract, issue invoices and comply with legal obligations. The legal bases are Art. 6(1)(b) and (c) GDPR.

This includes in particular:
• address details,
• communication data, such as telephone number and email address,
• data relating to the physiotherapeutic or osteopathic treatment of the animal, and
• documented therapeutic measures and treatment history.

Data is disclosed only where necessary for the performance of the contract, permitted by law, or separately requested or consented to, in particular to:
• pet registers, such as TASSO,
• pet insurance providers for claims assessment or reimbursement,
• other animal physiotherapy or veterinary practices in the event of a referral or therapeutic consultation, and
• external service providers acting on behalf of the practice for organisation, billing, communication or supportive therapeutic analysis.

Any further disclosure to third parties takes place only as permitted by law or with explicit consent. Further information, particularly regarding retention periods and data subject rights, is provided in the practice’s Privacy Policy.

Additional voluntary consent may be withdrawn at any time with effect for the future. Withdrawal does not affect the lawfulness of processing carried out before the withdrawal. Data required for contractual or legal purposes remains stored for the applicable statutory retention periods.

Treatment Agreement

I confirm that I am the owner of the animal and am therefore authorised to enter into an agreement for the required physiotherapeutic and osteopathic treatment. I further confirm that I am willing and able to pay the resulting costs.

If I am not the owner of the animal, I confirm that I am acting with the owner’s express authorisation. If such authorisation does not exist or is disputed by the owner, I agree to bear the costs arising from the treatment.

I confirm that the information I have provided is correct and instruct Vierbeinerphysio – Dr. Laura Klingseisen-Bruun to carry out the agreed examination and treatment of the animal. I confirm that I am authorised to do so.

I acknowledge that treatment fees are due immediately after treatment and must be paid in cash, by debit card or credit card. Follow-up treatments are also governed by this Treatment Agreement unless otherwise agreed.

The General Terms and Conditions (GTC) of Vierbeinerphysio – Dr. Laura Klingseisen-Bruun form part of this Treatment Agreement. They are available at https://www.vierbeinerphysio.de/agb/ and may also be inspected at the practice.

By signing, I agree to this Treatment Agreement and confirm that I have read the Data Protection Notice. I have indicated any additional voluntary consent or objections using the designated selection fields.
